Criminal History Reports

Please have these three (3) items with you when you come to the Calcasieu Parish District Attorney's Office to request your Criminal History Report:

1. A completed "Request for Criminal History Report" form. The form is attached to these instructions. If there is more than one person in your household that needs a report, then a form must be completed and signed by each person in your household.

2. Attach a copy of your driver's license or picture ID. Social Security Cards are not accepted.

3. Attach a $10 money order made payable to the Calcasieu Parish District Attorney's Office for each report requested. No cash or checks will be accepted.

MONEY ORDERS ONLY.

As soon as the report is ready, we will mail a copy to you and the agency requesting the report. Due to the number of requests we receive, you must allow ten (10) to fifteen (15) working days to complete your report. If you do not receive it in the mail after fifteen (15) working days then call our office.

If we run the report and realize you have outstanding charges (FUGITIVE STATUS), we can not mail out or fax the report until all charges have been taken care of by you.

If all of above instructions are not followed, it can cause a delay in the time that your request will be handled. Make sure all of the above instructions are followed carefully.
